Worms Ultimate Mayhem takes the series’ cartoon warfare into three dimensions, where destructible landscapes make the battlefield part of the strategy. Its turn-based structure gives players time to line up attacks and plan around terrain that can be deformed in real time. The arsenal mixes familiar choices such as the Bazooka, Dynamite and Holy Hand Grenade with stranger weapons, while custom-built weaponry lets players assemble surprises for their opponents.
There is plenty to do beyond head-to-head battles. The single-player offering includes missions, shooting challenges and speed runs, alongside timed tasks built around skills such as piloting a Jetpack or steering a Parachute. The game also brings together missions from Worms 3D and Worms 4: Mayhem, with voiced narrative sequences in its campaign. Teams can be personalized with names, accessories, voices and flags, adding a playful identity to the chaos.
For multiplayer, the game supports online and local matches, including offline play on one screen, with modes such as Deathmatch and Statue Defend. Custom settings and randomly generated landscapes add variety to those matches.
