UNDERWARD sends investigators into a deadly underground hospital to recover capsules containing «TEST-SUBJECT» monsters and carry them to an elevator. Its labyrinthine floors are randomized, and the unsettling setting can shift into areas that barely resemble a hospital. Along the way, players search for useful equipment while avoiding traps and enemies drawn to noise. The goal is not simply to find a route through the dark: the team must retrieve valuable cargo and meet a recovery quota before moving deeper.
Carrying a capsule makes noise and invites pursuit. A lone investigator cannot move the heavy capsule unaided, so multiple players can use the «Connected Carry» system to transport it faster. Cooperation also opens access to some areas and mechanisms, while proximity-based voice chat supports coordination under pressure. The game allows one to four players, and its single-player mode is described as an extreme stealth challenge, with detection leading to an instant game over.
The tension comes from a mix of hazards and shifting decisions: hide quietly when enemies are nearby, run if spotted, and weigh the risk of carrying noisy cargo against the need to recover it. Players can also exchange retrieval rewards for randomized items through the Supply Machine System. Customizable rules include respawns and time limits, while a total team wipeout means all is lost. These systems make UNDERWARD a cooperative extraction-style horror expedition where careful teamwork can matter as much as exploration.
