Pathologic 3 casts players as Daniil Dankovsky, a physician and researcher arriving in a remote town to investigate a man believed to be immortal. A deadly contagion overtakes the settlement, and the story sends Dankovsky back to the beginning after his failure to stop it. The central pressure is stark: he has only twelve days to save the town, while the mystery of death and the plague’s origins remains unresolved.
The crisis is approached through medical investigation and civic authority, not simply combat. Dankovsky meets patients face-to-face, examines them, and weighs conflicting accounts to diagnose their illnesses. His findings lead toward a possible vaccine, while decisions about quarantine, medicine, vaccination, patrols, curfews and unrest shape life in the town. Those choices carry consequences, and the people may come to hate the rules imposed on them. A shifting mental state also affects his actions and interpretation of events, making perception part of the danger.
Pathologic 3 is described as a single-player psychological horror game, with time itself built into its narrative design: players can rewind to revisit mistakes and pursue branching paths through moral dilemmas.
