Mad Island begins with a shipwreck: after a cruise boat sinks, two protagonists awaken on an unfamiliar island. Their goal is to find the friends who were aboard and escape. The story starts with the female protagonist, but players can switch between the pair at any time, giving the survival journey two playable perspectives.
Survival here blends combat and resource work with a settlement-like system. Players can gather, fish, farm, excavate, and craft at more than ten kinds of workbenches, making items such as weapons, protective equipment, clothing, and meals. Battles, excavation, and gathering contribute to leveling, which improves character attributes and skills. Island inhabitants and defeated or stunned enemies can also become allies; natives can be assigned jobs including logging, excavation, and farming.
Caves and ruins add a deeper exploration thread, with stronger bosses and powerful weapons farther in, alongside traps. The listed format is single-player, while the premise and systems center on managing the protagonists, allies, and island activities rather than coordinating a group of online players.
