Katawa Shoujo is a bishoujo-style visual novel set at Yamaku High School, a fictional school for disabled children in modern Japan. Its protagonist, Hisao Nakai, leaves his ordinary life behind after a congenital heart defect leads to a long hospitalization and a move to the school. The story follows his efforts to make friends and possibly find love in an unfamiliar setting.
The game presents its narrative through Hisao’s first-person perspective, using text and character sprites in a traditional ADV text box. Its structure branches into five main paths, each centered on one of the principal female characters and the story associated with her. That character-focused design makes the experience less about a single fixed sequence and more about following distinct narrative routes.
As a single-player visual novel, Katawa Shoujo is built around reading and character-specific storylines rather than shared sessions. The premise combines school life, relationships and the consequences of Hisao’s health condition, while the branching paths give the story its defining structure.
